Transitional Care

The Jewish Home’s Weinberg Manson Transitional Care Program is designed for patients who need short-term, transitional care after an illness or hospital stay for surgery.

We provide a range of therapy services to serve the special needs of adults. The transitional care program recently expanded from 25 to 35 beds and is designed to assist patients who must rebuild their strength and abilities before returning home. Admissions are available on a daily basis, including weekends.

  • Speech and language therapy
  • Orthopedic rehab
  • Bariatric services
  • Pain management
  • Wound care
  • Low-vision and functional-vision services
  • Dietary and recreation services
  • Incontinence therapy
  • Massage therapy
  • Tai chi and stress management classes
  • Social work services
